Community Based Service Provider

$43,000 - $46,000 annually

At CMCS (Cardinal McCloskey Community Services), our mission is to protect, empower and promote independence for at-risk children and families and those with developmental disabilities through quality community-based services.

As Community-Based Service Provider (CBSP) you will be responsible for direct service provision to children and their caregivers enrolled in the Children & Family Treatment Supports and Services (CFTSS) and Home & Community Based Services (HCBS) programs. The CBSP will be responsible for the provision of goal-driven, direct service provision, to assigned youth and their caregiver(s) as needed/assessed. Through a person centered planning approach, will engage the youth and their caregiver in developing their personalized treatment plan and identifying their future goals. Responsible for the successful completion of all treatment plans, safety plans and informational or service-related notes and collaborates with other providers to coordinate services that support the overall health and wellbeing of the child and family.

The successful candidate for the CBSP position will be a highly organized, independent worker who is able to track/monitor all service hours for a diverse caseload. This candidate must also be able to work using a multi-disciplinary approach and drive services from goals in treatment plan/Plan of Care.

Requirements

• Bachelor Degree in Human Services or related field and two years of experience working with children in a manner that supports their appropriate development and overall wellbeing.
• Understanding of mental health diagnoses, symptomatic behaviors and emotional response to trauma.
• Flexible schedule for direct service provision, which includes evenings and weekends.
• Bilingual Required English-Spanish
• Strong communication skills (oral and written)
• Strong technology skills.
• Effective engagement skills, creative and resourceful.
