Integrated Health 21 (IH21) is a leading provider of worksite wellness screenings and vaccination clinics across the continental United States. We are looking to increase our Health Coach staff – (Registered Nurses and Registered Dietitians) in the Chicago, IL area. We have several events coming up in Melrose, IL and encourage interested parties to apply.

This is a PRN, as-needed position.

PURPOSE OF POSITION:

This position will partner with clients seeking self-directed, lasting changes, aligned with their values, which promote overall health and wellness and reduce health risk. The position is responsible for providing effective health coaching services onsite or via telehealth. The Health Coach will work individually with clients to encourage, educate, motivate, and facilitate positive change in many areas including nutrition, physical activity, stress reduction, and empower participants to take an active role in the health.

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Engage participants who have specified risk factors in the collaborative development of a personalized behavior change plan to address risk factor(s) in conjunction with the physician or healthcare provider, when appropriate.
  • Ability and knowledge to triage employees who are at high physical or emotional risk to appropriate resources internally or through their employer.
  • Present to all participants with a positive and empathetic attitude to develop rapport.
  • Facilitate individual and group behavior change programs including health improvement programs, one-on-one health coaching, and meeting presentations.
  • Understand readiness to change and correctly refer to appropriate health enhancement programs and services.
  • Competently apply knowledge, skills, and training with participants regarding motivational interviewing techniques. Employ motivational strategies for encouraging individuals to explore motivation for change and enhance readiness.
  • Accurately document participant encounters in a timely fashion.
  • Track participant and program data for measurement of individual and program goal achievement, outcomes, and results using wellness platform and EMR systems.
  • Maintain accurate records and ensure confidentiality of data collected through the health assessments, onsite screenings and health promotion activities.
  • Understand and manage multiple company campaigns, simultaneously
  • Field general health related questions and concerns from program participants.
  • May participate in biometric screenings.
  • Follow all standard operating procedures in regards to Protected Health Information (PHI) in compliance with HIPAA regulations.
  • Ability to resolve participants or client concerns.
  • Execute other duties/projects, as assigned.

OCCUPATIONAL EXPOSURE:

  • Limited exposure to blood and body fluids while conducting biometric screenings. Employee is to wear personal protective equipment.

PHYSICAL DEMANDS :

  • Prolonged periods sitting at a desk and working on a computer
  • Ability to lift and carry up to 35 pounds

QUALIFICATIONS:

  • Bachelor’s Degree in Health Promotion, Nutrition, Nursing, Exercise Science or related field or equivalent work experience preferred.
  • Minimum 2 years of related experience in delivery and coordination of corporate based health programs.
  • Proficient knowledge of Microsoft products, specifically Excel.
  • Excellent verbal and written communication skills.
  • Strong organizational skills.
  • Problem solving ability to seek solutions using appropriate methodologies.
  • Ability to work effectively both independent and with team.
  • Maintain current certification/credentials.
