Internal Medicine Physician – Primary Care (House Calls)
Location: Brooklyn, NY
Employment Type: Full-time / Part-time
Pay Range: $220,000 - $250,000 per year, depending on experience (New York Pay Transparency)

We are seeking a compassionate, Board Certified or Board Eligible Internal Medicine Physician to join our team in providing personalized, in-home healthcare to our community. This role offers the unique opportunity to deliver comprehensive care in a home setting, with full support from an experienced team.

Position Overview
The Internal Medicine Physician will conduct house calls, managing a daily caseload of patients, and will focus on delivering comprehensive primary and preventive care services. This role is designed for a provider who values building strong patient relationships and delivering care where it’s needed most. Travel within the Brooklyn area is required.

Key Responsibilities

• Provide primary care and preventive health services directly in patients’ homes.
• Manage a daily caseload of patients.
• Conduct thorough assessments, create treatment plans, and provide follow-up care in a home setting.
• Collaborate with Medical Assistants, Nurse Practitioners/Physician Assistants, and administrative staff to coordinate patient care.
• Ensure accurate and timely documentation of all visits and treatment plans.

Requirements

• Board Certified/Board Eligible in Internal Medicine.
• Active licensure in the state of New York.
• Strong commitment to patient-centered, in-home care.
• Ability to travel within Brooklyn to conduct house calls.
