The ideal candidate must have a Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree and be Board Eligible or Board Certified in Pain Medicine and Anesthesiology. They should also have at least 1 year of post-fellowship experience and hold an active and unrestricted Medical License and DEA License for Arizona.
