Team Support: Be part of a collaborative and supportive team dedicated to excellent care. As a Clinical Care Registered Nurse (CCRN) you will: • Provide direct nursing care.
• Manage complex clinical situations independently.
• Collaborate with both licensed and non-licensed team members.
• Monitor and manage acute and chronic conditions in coordination with Primary Care Providers (PCPs).
• Administer treatments, procedures, and medications as prescribed.
• Conduct regular home visits for care, including wound care, lab draws, and other nursing tasks.
Qualifications • Bachelors degree in Nursing preferred; Associates degree required.
• RN-Nursing Diploma or Degree from an accredited School of Nursing or University.
• At least one year of experience working with the frail, elderly, or long-term care population.
• Experience in home care and wound care preferred.
• Current CPR certification by AHA or willingness to obtain.
Specific Requirements • Reliable transportation, a valid Michigan drivers license, and car insurance.
• Age 18 or older.
• Fully vaccinated, or willing to become fully vaccinated against communicable diseases such as Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B.
