What You’ll Do:

* Coordinate and provide skilled nursing care to home care clients across Westchester County.
* Perform initial assessments and create personalized care plans in collaboration with physicians and families.
* Support clients and caregivers with education to promote wellness and independence.
* Supervise and guide clinical staff to ensure the highest standards of care.
* Serve as a liaison between clients, families, and other healthcare providers to promote seamless care coordination.

What You Bring:

* Current NYS Registered Nurse (RN) license (CT license a plus!)
* CPR certification
* 3–5 years of nursing experience in home care or acute settings
* Strong leadership skills and a collaborative spirit
* Bachelor of Science in Nursing (preferred)
* Comfort with mobile documentation and electronic health records
