We provide support services and skilled nursing care to persons with intellectual and developmental disabilities, and the elderly. Description Family Model Provider (FMP) - Brentwood / Williamson County Position Summary
The Family Model Provider (FMP) provides daily care, support, and supervision for an adult with developmental disabilities in a shared living setting within the provider's home. This unique role allows you to make a meaningful difference by offering a family environment where the individual can thrive, build skills, and participate in community life.
Key Responsibilities
- Provide 24-hour support and supervision to the individual living in your home.
- Ensure health, safety, and emotional well-being in accordance with the person's Person-Centered Support Plan (PCSP).
- Support with daily living activities such as meals, personal care, medication administration (if certified), and transportation to community activities.
- Encourage independence, choice, and participation in community and recreational activities.
- Maintain clear, accurate documentation, progress notes, and incident reports as required by SimplyLove Care, MCO and DIDD/DDA standards.
- Collaborate with the care team, case managers, and family members to promote consistent, person-centered outcomes.
- Participate in ongoing training, monitoring visits, and quality reviews.
Qualifications
- Must be at least 21 years of age.
- Must pass all background, abuse registry, and reference checks.
- Must maintain a valid driver's license, reliable transportation, and car insurance.
- Must complete required DIDD/DDA and agency training, including First Aid/CPR, Medication Administration (if applicable), and behavior support.
- Ability to provide a safe, clean, and welcoming home environment that meets licensing and fire safety standards.
- Compassionate, patient, and committed to supporting individuals with varying needs and abilities.
